The Alcohol and Drug Abuse Prevention Team (ADAPT) is conducting a short survey in Taney and Stone counties. The survey will help us better understand adult behaviors and attitudes related to mental health and substance use. This survey will take approximately 10 minutes to complete and the information will help provide needed services and resources to our community.
No personally identifying information will be asked of you. You may skip any question or stop taking the survey at any time. If you complete the survey, you will be eligible to enter in a drawing to win one of five $30 Amazon gift cards. This is optional and an entry form will be provided after you complete the survey. Your contact information for the drawing is in no way attached to your survey answers.
